Administrative Hearing Request Form

If you have four (4) or more unpaid tolls with the PA Turnpike, or if you owe more than $250 in unpaid tolls and fees, your vehicle registration is eligible for suspension. To dispute the decision to suspend your vehicle registration, you must complete the following hearing statement within 30 days of receipt of your official notice of eligibility for registration suspension. You must also sign and date the hearing statement and include your contact information.

The opportunity to contest your vehicle’s registration is solely to dispute that the PA Turnpike notified you of your failure to pay tolls or fees. It is NOT to seek a reduction or dismissal of the amount owed or to set up a payment plan. You must contact the PA Turnpike’s Customer Service Center for that purpose. If you require assistance, please call the Customer Service Center at 877.736.6727 or visit https://www.paturnpike.com/pay-a-bill.

Failure to submit a timely hearing statement or failure to complete any portion of the hearing statement will be deemed a waiver of your opportunity to be heard and the suspension of your registration will take effect on the effective date listed in your notice. There will be no exceptions.

The PA Turnpike is authorized to take this action pursuant to 75 Pa.C.S. § 1380.

Claim of Unreceived Toll Violation Notices

I AM THE REGISTERED VEHICLE OWNER NAMED IN THE NOTICE OF ELIGIBILITY FOR REGISTRATION SUSPENSION. I DENY THAT THE PA TURNPIKE NOTIFIED ME THAT I FAILED TO PAY TOLLS OR FEES RELATED TO FOUR (4) OR MORE VIOLATIONS OF THE PA TURNPIKE’S TOLL COLLECTION REGULATIONS COMMITTED WITHIN A FIVE-YEAR PERIOD. SEE 74 PA.C.S. § 8117.

I REQUEST THAT THE PA TURNPIKE CONSIDER MY FOLLOWING STATEMENT AND ANY EXPLANATION ABOUT WHY I DID NOT RECEIVE SUCH NOTIFICATION, INCLUDING CONSIDERATION OF ANY DOCUMENTATION HEREIN ENCLOSED, BEFORE DECIDING ON MY CASE.

I UNDERSTAND THAT THE PENNSYLVANIA DEPARTMENT OF MOTOR VEHICLES MAY TAKE ACTION AGAINST ME, AUTHORIZED BY LAW. SUCH ACTION SHALL INCLUDE SUSPENDING MY REGISTRATION FOR THE LICENSE PLATE REFERENCED HEREIN.
